The letters SMF stand for Sick Mother Fucker. And the song was made for all the "hardcore" Twisted Sister fans. In fact, at shows, Dee usually says "this song is from us (TS) to you(fans)"

If you listen closely, the lyrics define all the hardcore TS fans.

One last thing is that usually the band plays SMF as a final encore song at shows.

anyone who didn't know isn't a real twisted sister fan. thats like asking what does s.m.f.f. of t.s. thats the original fan club its the sick mother fuckin friends of twisted sister. next someone will ask why love is for suckers sounds nothing like twisted sister. to answer before someone asks its a dee snider solo album pretty much put out under the twisted sister name with little input or playing from the rest of twisted sister which is why they never play any songs from it in concert now.
